Why use Accurx? This is a software company that healthcare providers and patients use to contact each other. It is already being used by patients to contact their GP practice about a health problem.
You can now access care from a range of NHS services directly, without the need for a GP appointment or GP referral.
When you submit an online GP request (through our website or the NHS App), you may be shown alternative options about other NHS services that you can access. This allows you to manage your health and get the answers you need quickly.
How to submit on online form to Moulton Medical Centre about a health problem
Please note! If your medical query is urgent then call the practice. If the practice is closed, contact 111 or for a medical emergency call 999.

Once you have clicked on the link the form will open, please check the details of the GP matches the GP practice you are registered with.

Next you will be prompted to select if the request is for yourself or someone else, then you can put the patients date of birth in.

Note: if you are accessing this form through the NHS app or NHS log in, the date of birth will be filled automatically.
Next you will be asked to confirm if the patient is experiencing any of the ‘red flag’ medical symptoms. 👇 The symptoms you see on this page will vary depending on the patients age.

If the patient is experiencing any of these symptoms or it's an emergency, do not continue with the form. Call 999.
If the patient isn't experiencing any of these symptoms, please confirm this, then you can describe the health problem. 👇

Depending on the health problem described, you might see some further information about when it might be more urgent and what to do, or other services or resources you can also access.
For example, if you are looking for help with a sore throat, you will be given information on how to find a pharmacy that can offer treatment and advice. This page also provides a link to NHS online advice.
